this case, Thomas Quinlivan M W 65, of 480 Prospect Place has beed
unconscious since time of occurance therefore the assigned has been unable
to interview him. The assigned interviewed Katherine Quinlivan of 628
Sterling Pl. who stated that she was the wife of the aided and that she
left him because he was an habitual drunkard. An interview with neighbor
Mr. Mcaneney of 480 Prospect Pl., Apt. 2R disclosed that subject at t/p/o
smelled of alcohol and had a partially emptied bottle of same along side of
him. Subject had been found face down at the foot of the concrete stairs to
his residence. Mr. Brown KCH states that subject suffered a right Frontal
Laceration of the brain. Request case remain active pending further

21, residing at 480 Prospect Place in the Brooklyn being duly sworn,
deposes and says: That she is a wife of the person whose body was found at
Kings County Hospital 11-28-63, and sub- sequently sent to the City
Morgue; that deponent has seen the body of said deceased, and has every
reason to believe that the body now recorded at the Morgue as Thomas
Quinlivan is Same who was last seen or heard from by deponent on Wednesday.
Deponent therefore prays that her identification of said deceased person be
accepted by the Chief Medical Examiner of The City of New York. Age: 64

Nov. 22, police took him to LIC Hospital. Found unconscious outside

Years Approximate Weight: 165 Pounds Height: 67 Inches
Identified by Catherine Quinlivan - Wife Residence 480 Prospect Place,
Brooklyn, N.Y. Stenographer: Elaine Rutman Book #3 I hereby
certify that I Dr. Nathan B. ludwig have performed an autopsy on the body
of Thomas F. Quinlivan at Kings County Mortuary on the 29th day of
November, 1963 and said autopsy revealed: Body is that of an adult, white
elderly male, 67 inches in length, weighing 165 pounds. Head shows evidence
of surgical perforation, with a skin flap over the right parietal region,
which is almost completely healed. There is also an almost completely
healed surgical wound of the left parietal region. There is peri orbital
hematoma of the right eye. There is sutured tracheostomy wound of the neck.
Bilateral multiple contusions of both chests anteriorly, as well as
multiple contusions of both arms, forearms. Remainder of the chest, abdomen
and extremities, external genitalia is normal. HEAD: On reflection of the
scalp in the usual manner, and removal of calvarium, opening of dura,
removal of brain, reveal the following: Galeal hemorrhages of the right
temporal regions. Hemorrhage in the right temporal muscle. A segment of the
right temporal bone absent, 1'x 1 1/2', surgically. There are
multiple lineal fractures of the right frontal, right temporal, as well as
fracture of the supra orbital plate on the right side. The fracture line in
the right frontal line shows one major limb extending down to the posterior
portion of the greater wing of the shenoid. The fracture line of the
temporal bone extends for a distance of 3' medially, and anteriorly on the
squamous portion of the right temporal bone, as well as the secondary lines
of fracture, which extend into the petrous temporal. There is a subdual
hematoma, encircling the left cerebral hemisphere of 150 grams. Section of
the brain, reveals multiple subcortical and intracerebral hemorrhages
varying from 2 to 5mm., as well as a laceration of the right
temporal-parietal regions. Brain weighs 1400 grams. ON SECTION:
Cavities: Normal Cario Vascular System: Heart weighs 390 grams.
Epicardium, myocardium, endocardium normal. Coronary arteries show
segmental atherosclerosis. Valve leaflets are thin. Respiratory System:
Trachea and bronchi shows tracheostomy, lower thyroid cartilege.
Tracheo-bronchi contain a moderate amount of blood tinged mucous. Lungs
weigh 1100 grams combined. Cut section shows moderate congestion and edema.
Gastro Intestinal System: Normal throughout. Hepatobiliary System: Liver
weighs 1900 grams. Cut section shows a hemo- geneous red-brown surface,
without evidence of lobulation. Gall bladder is thin walled, devoid of
bile; no calculi. Pancreas: Weighs 75 grams, shows lobulated tan fatty
surface on section. Spleen: Weighs 120 grams. Cut sections show a soft
red-purple pulp. Genitourinary System: Right kidney weighs 150 grams. left
kidney weighs 140 grams. Capsule strips easily. Cut section shows normal
cortico-medullary markings, with slight increase in peripelvic fat.
Remainder of the urinary collecting system is normal. Prostate, Adrenals,
Thyroid: Normal. Cause of Death: Skull fracture; Brain Laceration;
Hemorrhage: Pending Police Investigation **************

Symptoms: Pt. brought to hosp. from LIC hosp. has history of being found
unconscious by policemen; was somewhat alert at LICH but was stuporous when
seen at KCH. No history obtained. Had left hemiplegia. Pt. began to
deteriorate, and pupils became fixed. Pt. decerebrate and breathing
irregularly. Had rt. front temporal swelling and rt. periorbital
lechymosis. Pt. found unconscious at unknown place - no history is
available. 11/23/63, Kings County Hosp., tracheotomy - improved resp.
11/23/63, Kings County, burr holes, rt. frontal cranietomy - evacuation of
blood. Death took place on the 28 day of November, 1963, at 10:15 o'clock
